What insurance covers tubal reversals?

I know I have a co-pay for the testing to see if my tubes are still tied after 17 years, but I'm not sure if my insurance will cover the procedure. I haven't asked that yet because I just want to take it one step at a time. As for my Dr asking about my husband, there is consultation involved before the test -- its some kind of dye they insert into your tubes. I mean what if I were single and just wanted this done should I get a husband. I am married but what if I weren't? Does a Dr have the right to deny treatment without a mate involved?
